
A long term health care organization with nearly 3000 members will cease operations at the end of the year. Community Health Partnership, Inc will cease operating the Partnership and Family Care programs as of Jan 1, 2013. The 2700 people affected by this will have help coming from the Wisconsin Dept. of Health Services.
DHS will contract with at least one, if not more new managed care organizations to provide help to members starting in 2013. Members served cover several counties: Eau Claire, Chippewa, Dunn, Pierce, and St. Croix counties.
CHP is based in Eau Claire and provides care for older people and those with physical or developmental disabilities.
